Please carefully read and consider the terms of this agreement before registration of your child (the “participant”) for the Humblebee’s Toiletries Co. (DBA Beauty Sweet Workshop) event. By signing the agreement, the undersigned agrees to be bound by its terms.
1. Humblebee’s Toiletries Co. provides its services, including hands-on events and demonstrations (“event”), using a reasonable level of skill and care. Humblebee’s Toiletries Co. (DBA Beauty Sweet Workshop) shows participants how to work with tools, supplies, and ingredients safely. Participants may work with synthetic, and natural, raw materials used in making cosmetics and crafting slime during the event. The goal is for each participant to have fun and learn something new. However, participants could have allergies or sensitivity to items used in the event. If undersigned knows of any allergies of participant, the undersigned should ask questions, assess the risk, and determine whether the the participant should attend the event.
2. Limit Of Liability/Own Risk; Applicable Law. To the extent permitted by applicable laws, all participants attend the event at their own risk. The undersigned further expressly agrees that this agreement is intended to be as broad and inclusive as is permitted by the law of the state of Illinois. If any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.
3. Release. Humblebee’s Toiletries Co. shall not be liable for, and the undersigned hereby releases and shall indemnify and hold Humblebee’s Toiletries Co. (DBA Beauty Sweet Workshop) harmless from, any and all liability to the undersigned or participant, including claims by all his/her representatives, assigns, heirs, and next of kin for any loss or damage, and any claim or demands, therefore, on account of injury to the person, including death, or damage to property of the undersigned or participant, whether caused by the negligence of Humblebee’s Toiletries Co. or otherwise, while the undersigned or participant is in, upon, or about the premises or any facilities or equipment therein or participating in any event affiliated with the Humblebee’s Toiletries Co. (DBA Beauty Sweet Workshop).
4. Photography. The undersigned, on behalf of the participant, assigns the irrevocable and unrestricted right to use and publish video and/or still photographs of the participant taken during the event, or in which participant may be included for any lawful purpose, including promotion, advertising, information, and in any manner and medium, including social media; and to alter and composite the same without restriction and without the inspection or approval of undersigned. Undersigned releases Humblebee’s Toiletries Co. (DBA Beauty Sweet Workshop) and its legal representatives and assigns from all claims for compensation and liability relating to said photographs.
I am the parent or legal guardian of the participant. In consideration of allowing the participant to take part in the event, I voluntarily execute this agreement on his/her behalf. I state that the participant is physically able to participate in the event. I have carefully read this agreement, and i understand its contents.
I understand that this is a release and waiver of liability and a contract between myself and Humblebee’s Toiletries Co. (DBA Beauty Sweet Workshop), and I sign it voluntarily and on behalf of the participant named below. No oral representations, statements or inducement outside of this written agreement have been made.
